
Spravato (esketamine) is an FDA-approved prescription nasal spray used alongside an oral antidepressant for adults with treatment-resistant depression (TRD) and major depressive disorder (MDD) with suicidal thoughts. Because of potential side effects like sedation and dissociation, it must be administered in a certified medical setting under supervision.
Spravato is not identical to traditional ketamine, but it is a direct chemical derivative of it. Specifically, Spravato is the brand name for esketamine, which is an isolated portion of the ketamine molecule.
Unlike traditional antidepressants that target serotonin, Spravato targets the brain's glutamate system (specifically NMDA receptors). It is fast-acting and can sometimes provide relief within hours or days.
Temporary side effects during treatment include dissociation (a floating or disconnected feeling), dizziness, nausea, and a temporary increase in blood pressure. It is required for you to be monitored by a healthcare professional for at least two hours after dosing.

Spravato is FDA approved and covered by most insurances. Your cost should be whatever your typical copay is for an office visit. Spravato also has great rebate and savings card programs. Most of our patients, with the rebate, end up paying $0. Even patients with a high deductible plan can take advantage of the generous rebate (we will file all the paperwork for you), and find treatments are an easy way to "drive down" their deductible. Our staff will be happy to look into your benefits for you and answer any questions you have.
Spravato causes sudden sedation, dissociation, and impaired motor skills that make operating a vehicle highly dangerous. Even if you feel completely alert, the medication temporarily alters your perception, judgment, and reaction times. You must not drive, ride a bicycle, or operate heavy machinery until the next day following a restful sleep. We also recommend no online shopping until the next day.
Yes. We ask that you "share" the ride location with our office staff so we can confirm you arrived safely. We are also happy to arrange pickups and return rides for you, and the cost will be invoiced to you.
You will be seated in a comfortable recliner. We will have relaxing music or sounds playing softly. You are welcome to bring anything that will make you more comfortable, headphones, eyeshades blankets, stuffed animal, etc
You are allowed to have someone sit with you during treatment if you prefer. We just ask that you be quiet to respect others that may be having treatment.
Typically, no. Most find it relaxing. It can be intense for some people, especially those with past trauma or PTSD. Often this can ultimately turn out to be healing for those patients. If you have a history of abuse, trauma, PTSD, you may want to consider having one of our therapists sit in on your session. You are not required to talk, but some find it comforting just having someone there with them.
Spravato is a medical treatment. It is not a spa treatment or just a chance to "get high." While dissociation can occur, it is not required for the treatment to be effective.
